What I am signing up for

I am voluntarily taking part in a group fitness session hosted by Foster Physique LLC and, where listed, a co hosting coach or partner brand. Sessions may include strength training, conditioning, running, jumping, swimming, skating, outdoor work and use of gym equipment.

I understand the risk

Physical training carries real risk. That includes muscle strains, sprains, fractures, heat illness, dehydration, drowning where water is involved, falls, equipment failure, cardiac events and in rare cases serious injury or death. I accept those risks knowingly and voluntarily, including risks that come from ordinary negligence in the running of the session.

I am medically cleared

I confirm I am in good enough physical condition to take part, that I have disclosed below any injury, condition, medication, pregnancy or limitation a coach should know about, and that if I have any doubt I have spoken with my doctor first. I will update the coach if anything changes before the session.

I will train within my limits

I will follow coaching instruction, scale or sit out any movement that does not feel right, and stop and tell a coach immediately if I feel pain, dizziness, chest tightness or shortness of breath. I understand every movement can be scaled and that no one will push me past what I say I can do.

Release

In exchange for being allowed to take part, I release and hold harmless Foster Physique LLC, Malcolm Foster, any co hosting coach or partner brand named on the session, the owners and staff of the host facility, and their employees and volunteers, from any claim, demand or cause of action arising out of my participation, to the fullest extent allowed by Florida law. This release binds me, my family, my heirs and my estate.

Emergency care

If I am injured or become unwell, I authorize the coaching staff to arrange emergency medical care on my behalf, and I accept responsibility for the cost of that care. I have given an emergency contact below.

Photo and video

Sessions are sometimes photographed or filmed for the coaches' own social pages and websites. I grant permission to use my likeness in that content without payment. If I would rather not appear, I will tell a coach before the session starts and my wish will be honored.

Conduct and property

I will treat the facility, the equipment and everyone in the room with respect. I am responsible for my own belongings. A coach may end my participation in a session for unsafe or disruptive behavior.

My signature

I have read this in full. I am at least 18 years old, or a parent or legal guardian signing for a participant under 18. I am signing freely and I intend my typed name below to be my legal signature, with the date and time recorded automatically.
